Skarbinski, Jacek

Jacek Skarbinski, MD, is a research scientist at the Kaiser Permanente Division of Research and a practicing infectious diseases and HIV medicine specialist at Kaiser Permanente, Oakland Medical Center. In addition, he serves as the Program Director of the Kaiser Permanente Northern California HIV Fellowship. He is part of The Permanente Medical Group Physician Researcher Program and serves as co-chair of the Kaiser Permanente Northern California COVID-19 Clinical Research Coordinating Team.

Before joining The Permanente Medical Group in 2016, Dr. Skarbinski served with the Centers for Disease Control and Prevention (CDC) in various epidemiology and leadership roles. His CDC research addressed the epidemiology, diagnosis, treatment and prevention of infectious diseases with a focus on malaria, HIV, tuberculosis, and influenza. His current research interests include clinical trials, observational studies, and implementation science in a broad range of infectious diseases including HIV, tuberculosis, and COVID-19.

Dr. Skarbinski earned his MD at Stanford University School of Medicine after completing his BA at Cornell University. He completed his internal medicine residency at the University of California-San Francisco in the San Francisco General Internal Medicine Primary Care Track, served as an Epidemic Intelligence Service Officer at the CDC and completed an infectious diseases fellowship at Emory University School of Medicine.

California Emerging Infections Program Population-Based Influenza Hospitalization Surveillance

The goal of the project is to determine the age specific rates of laboratory-confirmed influenza associated hospitalization among children and describe clinical and epidemiologic characteristics of pediatric hospitalized case-patients during the influenza season.

California Emerging Infections Program FoodNet

Foodborne Diseases Active Surveillance Network (FoodNet) is a sentinel surveillance network that produces national estimates of the burden and sources of specific foodborne diseases in the United States through enhanced active, laboratory-based surveillance and additional studies.

California Emerging Infections Program Respiratory Syncytial Virus (RSV)

The California Emerging Infections Program (CEIP) is collaborating with KPNC on prospective and retrospective collection of Respiratory Syncytial Virus (RSV) data for reporting to the Centers for Disease Control and Prevention (CDC). Children less than two years old hospitalized with a positive RSV test within 7 days of admission who reside in Alameda, Contra Costa, or San Francisco counties will be identified by data queries and a weekly report will be delivered to the CEIP RSV surveillance coordinator via encrypted email. Medical chart reviews of all identified laboratory-confirmed cases also will be conducted to collect relevant clinical and epidemiologic information. Surveillance will be conducted at all hospitals that admit pediatric patients within the CEIP catchment area.

California Emerging Infections Program COVID Surveillance

The primary aims of this study are to estimate age-specific hospitalization rates in a timely manner; describe the temporal trends of laboratory-confirmed SARS-CoV-2 hospitalization; and utilize hospitalization rate data to estimate the national disease burden of COVID-19.

California Emerging Infections Program Surveillance to Optimize Protocols for Early Identification and Subgrouping of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS)

This 5-8 year project involves conducting active, syndromic surveillance of prolonged, unexplained fatigue, to advance the understanding of the development and early stages (first 2-5 years) of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome ME/CFS. We will begin by developing methods and a protocol for finding patients with unexplained, prolonged fatigue that interferes with their ability to perform daily activities and following these patients to document future diagnoses. We will then conduct prospective surveillance to better understand the early stages of ME/CFS.

Reducing infectious complications in patients with acute myeloid leukemia

Currently available treatments for acute myeloid leukemia, uniformly lead to severe and prolonged periods of neutropenia and a substantial risk for infectious complications. Disruptions to the mucosal barrier, presence of implanted lines, and the immunomodulatory effect of the malignancy itself also contribute to the risk for infectious complications. Mortality rates for febrile neutropenic episodes range between 4% to 20%. A top priority for our KPNC regional leukemia and lymphoma treatment centers is to reduce infection-related deaths by optimizing prevention, diagnosis, and treatment for these high-risk patients. Data produced in this study will standardize key aspects of care across KPNC and inform national guidelines.

Sexual health e-visit as a tool to improve access to prevention and treatment of sexually transmitted infections

This study will explore the role the sexual health e-visit plays in members’ access to sexual health services. The findings will inform future e-visit development within Kaiser Permanente Northern California and the use of similar platforms in other health care systems.

CEIP Post COVID Fatiguing Illness

This study aims to identify and characterize persons who are at risk of developing or who have developed post-COVID conditions, including those with medically unexplained illness in certain risk groups, and identify risk factors for post-COVID conditions.

Monitoring shared variation in genomes of SARS-CoV-2 among persons with COVID-19 with and without vaccination in Northern California

We are studying the genomes of SARS-CoV-2 in persons with COVID-19 with and without vaccination to identify and describe variants circulating within our patients, including variants of concern (such as Delta or Omicron. We also are assessing which shared genomic variations are associated with a greater risk for infection (symptomatic and asymptomatic) and severe outcomes (such as hospitalization, intensive care unit use, and death), and whether certain subpopulations, such as those with certain medical comorbidities, might be at a greater risk for infection and severe outcomes from identified variants. Through modeling, we are investigating whether and which mutations result in decreased vaccine effectiveness for prevention of infection and severe outcomes, and which may require a booster or an updated vaccine.

Tuberculosis Epidemiologic Studies Consortium III

This study will identify primary care systems and settings that serve non-U.S.-born persons at risk for latent tuberculosis infection (LTBI) and tuberculosis disease; collect retrospective and prospective electronic medical record data; design and implement clinical care-based interventions to improve performance measures across the LTBI care cascade; and monitor and evaluate intervention performance over time to identify efficient and effective strategies.

Comparing the frequency of SARS-CoV-2 variants of concern among vaccinated and unvaccinated COVID-19 cases

In partnership with the California Emerging Infection Program and the California Department of Public Health, Kaiser Permanente Northern California will contribute to statewide whole genome sequencing efforts for surveillance for SARS-CoV-2 variants of concern by submitting samples from all vaccine breakthrough cases, a subset of hospitalized cases, and a set of matched controls. Sequencing will be done in a central laboratory and sequence data reported to public health authorities.

This study will assess the sero-prevalence and sero-incidence of SARS-CoV-2 infection in Kaiser Permanente Northern California and assess correlates of exposure and immunity among KPNC members.
