Habitat Health Implementation and Impact Evaluation

The Habitat Health program uses the Program of All-Inclusive Care for the Elderly (PACE) model and will be piloted in Sacramento and Los Angeles in 2025. This quantitative evaluation project will provide rigorous comparative data related to quality of care, utilization, and patient characteristics to provide high-quality information to evaluate the impact of the program relative to usual care.
